Overview
Reach Out is a full-service referral agency serving low-income San
Diego County residents who have medical need and no healthcare
coverage. Callers are referred to primary care physicians,
specialists, and ancillary services (vision, dental, counseling
etc.) after an assessment is made to determine the client is not
eligible for coverage through other programs such as County Medical
Services (CMS), Medi-Cal, Veterans Benefits, etc. Providers discount
fees charged for the services in exchange for cash payment from the
patient at the time the service is provided. Reach Out has been
formally endorsed by the San Diego County Medical Society.
Eligibility & Access
All San Diego County residents can call Reach Out and receive
services. Clients can self refer or can be referred by medical or
non-medical personnel.
Funding
Reach Out is primarily funded
through grants and private donations. Reach Out does not charge fees of
any kind for the services it provides to callers.
Services
Reach Out links uninsured/under-insured callers, with a medical
need, with physicians and medical services including limited
pharmacy and laboratory procedures.
Accomplishments
With over 600,000 uninsured/under-insured residents in San Diego
County today, Reach out increases access to care for those who "fall
between the cracks", those who frequently forego medical care until
they are forced to utilize hospital emergency room treatment. Reach
Out provides services to over 2,000 patients annually through a
network of caring physicians and providers. |
